Help seekers with overlapping social and health needs who are having difficulty accessing services can reach out to the Community Care Hub Intake Center for local support and care coordination. We help people find resources, apply for assistance, and access interpreter services in over 155 languages.

Crisis Connections is the intake center for community care hubs across Washington state. Comprehensive, wrap-around services are at the heart of our mission to support whole person care.

Reasons people call

People and families experiencing a combination of health or social needs can call the intake center to get connected to a local community-based worker for care coordination assistance. Concerns may include health challenges, difficulty accessing healthcare or benefits (SNAP, WIC, Apple Health), housing, food, and childcare.
